Q: How do I get my private rating?

A: It's so easy! Print and complete the referral request form. Mail or fax the request with Medical Reports. Analysis should be completed within 10 business days. Payment should be mail with request. If prior arrangements are made, I will invoice as well.

Q: How can I sign up for your classes?

A: It's simple, print out the enrollment form, complete and mail to me with your payment.

Q: What do I need to bring to the class?

A: You need to bring a copy of the schedule and the AMA guides. The schedule can be printed from the DWC website and the AMA guides can be purchased from the AMA online bookstore.

Q: Do I have to take the classes in order?

A: No the modules are independent from each other and can be taken in any order.

Q: What are the charts and handouts?

A: The handouts for each module are the individual sections of "The Ultimate Rating Guide II " - which include Spine, Upper Extremities, Lower Extremities, and Other body parts - Each section is filled with charts, directions, examples and answers. Once you complete all four modules, you will then have the complete "Ultimate Guide II".

Q: Can I just show up , or do I have to pre-register with my payment?

A: Walk-in's would be accepted based on availability of seats. However, as my last series of classes sold out, I would recommend sending in your enrollment form as soon as possible.

Q: Do you take credit cards?

A: No, at this time I am only able to accept checks and /or money orders.

Q: I want a private rating class for my firm in my office, how can I arrange that?

A: Give me a call 661-313-3768.

Q: I've already signed up for one of your classes, but now I can't go, what are my options?

A: You can assign a substitute to attend in your place, or with prior notice and at my discretion I will give a class credit to be used for a future date. For class credit you must contact me prior to the class.

No Show with No Notice = No Credit.

Q: Do you offer independent rating services?

A: YES! I am pleased to announce that I am now able to offer my skills as a private rater. Click on private rating link to the left for additional information..
